APPLY TO FREE2TALK

Please complete the application in order that Free2Talk may determine whether the patient qualifies to receive financial support from Free2Talk (up to 80% of costs for up to 16 sessions) to offset the cost of mental health or speech therapy. 

Such therapy will be provided by one of the qualified mental health or speech therapy professional partners listed here. Free2Talk will provide directly to the provider on a monthly basis 80% of the cost of treatment, after Free2Talk receives the invoice from the provider. The patient will pay 20% directly to the provider within the time required by provider.  If patient fails to pay partner practice on a timely basis, Free2Talk will terminate assistance.
